To make an appointment at the UCSF Audiology Clinic, please call (415) 353-2101. If you have not had a hearing evaluation in the past six months, please notify the receptionist when making your appointment so sufficient time can be scheduled. Please inform the receptionist if you're involved in any legal action, as we may not be able to accept you as a patient.
During your appointment, you will learn about tinnitus, including treatment options. Many of these management procedures are not intended to cure tinnitus but are designed to help you cope with tinnitus and develop strategies to best adapt to the symptom. Please be aware that the success of any tinnitus management approach depends on your active participation.
Please fill out our Tinnitus New Patient Information Form (PDF) and Tinnitus Handicap Inventory (PDF). Print, fill out and bring the forms with you, fax, or mail them to us before your appointment. Bring any audiological records that you have to your appointment. Read more information about becoming a patient and your first appointment.
