The UCSF Balance and Falls Center is located at 2380 Sutter Street on the first floor.
To make an appointment at the center, please call (415) 353-2101. Prior to the balance assessment, you must have had a hearing evaluation by an audiologist in the past six months.
Please read our Patient Instructions and complete our Test Overview, Dizziness Handicap Inventory and Vestibular History forms. Fax or mail all completed forms to the center, along with any prior audiologic test results.
