Appointments
To make an appointment, please call the clinic where you'd like to be seen. If your insurance plan requires, you may need a referral from your primary care physician or the doctor currently treating you or an "authorization" from your insurance company to make your first appointment.
After scheduling an appointment, complete the physician contact information form, to provide information about other doctors treating you. Please bring the form with you to the clinic. If you have any questions regarding the form, please contact the clinic.

Your First Appointment
We recommend that you arrive 15 minutes before your appointment and that you bring a family member or friend to help find your way, take notes and for moral support. If you're unable to make your appointment, please contact our office as soon as possible.
Please complete the physician contact form with information about doctors treating you and bring it with you to your appointment. In addition, please bring copies of all your insurance cards.
If you have any special needs, please let us know in advance. If you don't speak English well or if you're speech of hearing impaired, we can arrange for an interpreter at no cost to you.

Health Insurance
Before your appointment, please review your insurance benefits. Call your insurance company or consult the insurance information provided by your insurance carrier. Each insurance plan has rules that must be followed to receive full benefits.
If your insurance plan requires prior authorization for office visits to a specialist, please fax referrals and authorizations to the clinic before your appointment. Otherwise, you will be considered a "self-pay" patient and will be responsible for all charges associated with your appointment.
Deductibles — If your insurance plan has a deductible, UCSF will first bill your insurance company. If you have not yet met your deductible, you will receive a bill for the medical services you received. The bill will reflect a reduced rate defined by your insurance company's contract with UCSF.
Health Plan Savings Account — If you have a health plan savings account with a card, please contact your insurance company to activate your card and notify the company that you'll receive care at UCSF. The process typically takes 10 to 14 days.
Same-day Procedures — If your doctor recommends a procedure, such as a biopsy, to be performed during the day of your appointment, there will be an additional charge for this service, which can range from $1,500 to $10,000. Please discuss pricing in advance.
Second Opinion — If you're seeking a second opinion, your UCSF doctor may ask that your existing pathology slides and radiology films are reviewed again at UCSF. The cost of these evaluations can range from $400 to $2,500. You may be asked to make a deposit for these reviews in advance. If you'd like a cost estimate prior to the evaluation, please contact:

Self-pay Patients — If you don't have insurance or you choose not to use your insurance, a deposit of $345 will be collected when you check in to see the doctor for the first time. This fee covers your first exam. If your doctor performs any tests or procedures, you will be billed for those additional services. The deposit fee for future follow-up visits is $160. UCSF provides a 30 percent discount to self-pay patients.

Medical Records
The review of your medical records is an important part of your evaluation. (You may be asked to fax your records to the clinic before your appointment is scheduled.) Please fax your records to (415) 353-7657 for Dr. Lee-May Chen or Dr. Bethan Powell or to (415) 353-4298 for Dr. John Chan.
Please provide the following:
- Copies of all records relevant to your medical condition, including medical records from doctors who referred you. This may include records from your primary care physician, gynecologist, oncologist or surgeon.
- Pathology reports of any biopsy or test you had done. If you are seeing a surgeon, please also bring actual glass pathology slides.
- Any X-rays, mammograms, magnetic resonance images (MRIs) or any other films that relate to your medical condition and written reports that accompany those films.
Social Work Services
Social workers are available to meet with you and your family to discuss financial issues, such as short- or long-term disability income, insurance coverage and special programs that assist people with cancer; support groups and referrals to a psychologist or chaplain; family issues such as care giving and talking to children about cancer; and lodging and transportation.
