Screening for Bariatric Surgery
To be considered for surgery at the UCSF Bariatric Surgery Center, the following documents are required for initial screening.

Referral Letter -- A letter of referral from your primary care doctor. This letter should include a summary covering the past two to three years of your diet and weight history and a summary of your medical conditions.

Insurance Requirements -- A copy of your current insurance card, front and back of the card, as well as a copy of your insurance company's criteria and requirements for bariatric surgery. your primary care doctor should request this from your insurance company.

Insurance Authorization -- An authorization or referral from your insurance company, if required, for an initial visit with a bariatric surgeon. Health maintenance organizations (HMOs) and Medi-Cal managed care insurance plans require an authorization or referral to be seen as a new patient at the UCSF Bariatric Surgery Center.
The information above is required in addition to the new patient questionnaire.
Please submit all requested documents together. Partial information will not be reviewed.

The new patient questionnaire, letter of referral and insurance documentation will be reviewed by our bariatric surgery team. You will be contacted by a member of the team to determine your eligibility based on the information provided.
To qualify for bariatric surgery, you must weigh less than 450 pounds because our hospital X-ray equipment can't accommodate patients who weigh more. A nutritionist is available to help you lose weight to meet this requirement, if necessary.
