If you have a personal doctor, please request a referral to see a cardiologist at UCSF Medical Center. Your personal doctor has your medical history, from the medications you take to the upset stomach you had four years ago. Working with your personal doctor will help our team to learn more about you and your health.

Bring your medical records or request that your doctor send them to the cardiologist. It's important that our doctors can look at X-rays, test reports and notes from previous health care providers. Most doctors and health care organizations will forward your reports to us if you ask.

Other tips for your appointment:

• Directions -- Examine travel routes and, if necessary, reserve lodgings. If you're not familiar with San Francisco, please refer to our directions section.

• Medications -- Bring the medicines you are currently taking in their original containers, including over-the-counter or non-prescription medications and vitamins. This is better than a list of medicines.

• Questions -- Make a list of your questions. We want to make sure we address all of your concerns.
