The UCSF Memory and Aging Center is located on the seventh floor of the medical office building at 350 Parnassus Ave., Suite 706 in San Francisco.
To make an appointment, please call (415) 476-3898. You will be asked to provide medical records from your primary care doctor or neurologist before an appointment is scheduled. If your health insurance is covering the fee for this appointment, you may be required to obtain an authorization from your insurance company.
