The UCSF Multiple Sclerosis Center is located in the Ambulatory Care Center at our Parnassus facility. Many health insurance plans require that you obtain a referral from your primary care doctor before you make an appointment with our MS specialists. Please contact your insurance plan to find out if you need authorization to make an appointment.
Before your appointment, please ask your primary care doctor or neurologist to fax or send your medical records, including magnetic resonance imaging (MRI) films. Please arrive 15 minutes before your appointment to complete patient forms. In addition, bring your insurance card and any authorization documents your insurance plan provides you.
