Appointments
To make an appointment, you need a referral from your personal physician. Your physician should call the UCSF Heart and Lung Transplant Program at (415) 353-4145. A history, physical examination and recent test results will be requested. Upon receipt, the information is screened and discussed with the medical director and an evaluation is scheduled with the Lung Transplant Team.
The Lung Transplant Office requests the following information of the referring physician or institution:
- Recent clinical summary, including all current medications and treatment plans
- Recent labs
- All demographic and insurance information
- Recent EKG, chest X-rays
- Results of gynecological exam with pap smear within past year
- Mammogram for females over 40
- PSA level for males over 40
Potential lung transplant candidates are evaluated by the Transplant Team, which includes the transplant surgeon, pulmonologist, clinical social worker, a transplant nurse coordinator and a physical therapist. The team reviews the consults, recommendations and testing results, and makes decisions about a patient's suitability for transplant. The referring physician is provided with a written summary of the evaluation and selection process.
