We encourage all patients to complete an advance
health care directive, which allows you to state your preferences
for medical treatments and to select an agent or person
to make your health care decisions in case you're unable
to do so or if you want someone else to make decisions for
you.

If you already have an advance health care directive,
please bring a copy with you when you're admitted to the
hospital so you can discuss your wishes with your health
care team. Your advance health care directive will be placed in your
medical chart.
