To refer a patient, contact a doctor or department directly or call our Referral Liaison Service for assistance. Please use our referral forms to provide information about your pediatric or adult patients. See the links below for a list of services as well as phone and fax numbers.

Referral Liaison Service

Our Referral Liaison Service helps provide access to our doctors and medical services. Referral liaisons can expedite the process, help you obtain follow-up information and help resolve difficulties you may experience. Referral liaisons also are available to visit referring doctors, medical groups and health plans to provide more information.

Health Plan Services

UCSF Medical Center and UCSF Children's Hospital have contracts with many preferred provider organizations (PPOs) to provide comprehensive inpatient and outpatient care. We also have contracts with many health maintenance organizations (HMOs) to provide specialized inpatient and outpatient care.

If there is a contract in place, please follow the authorization procedure required by the plan when referring patients to us.
