Appointments
Information about family members, including names, addresses, telephone numbers, dates of birth and past medical histories will be needed during the evaluation appointment. Also bring your insurance information, including the insurance company name and policy number. Your referring physician should send a copy of your child's medical records to the transplant service before your appointment.
You and your child will be interviewed by a nurse specially trained in kidney transplantation, a kidney transplant physician or surgeon, a social worker and a financial counselor during the evaluation appointment. If possible, have family members or close friends accompany you to the appointment to help you understand the significant amount of information you will receive about the transplantation process.
Decisions about accepting your child for our program are made by the transplant team.
