If your child is approaching end stage renal disease or is currently on dialysis, you can learn more about kidney transplantation by making an evaluation appointment. All evaluation appointments, including those at outreach clinics (except for Hawaii), are scheduled by calling (415) 353-1551 and selecting option #3. Please be prepared to provide basic insurance information. If you are calling from outside the Bay Area, we can be reached at 1 (800) 4UCSFTX.

You, your referring physician or a staff member of your dialysis unit can make the appointment. You also may send a letter requesting an appointment to the transplant service address.

Once your child's evaluation appointment is scheduled, you will be sent directions and information about parking and convenient short-term lodging.
