Audiology Clinic

At the UCSF Audiology Clinic, we treat hearing loss and related auditory problems in individuals of all ages. The clinic works in close cooperation with a variety of specialists to provide state of the art diagnostic, electrophysiologic, and balance testing for patients with any degree of hearing loss or communicative impairment. A full-service dispensing program is available for hearing aid and assistive listening devices as well as rehabilitative services for hearing loss, dizziness and balance problems, tinnitus and hyperacusis.

The procedures offered by our audiology program include:

  • Assistive listening device dispensing
  • Auditory evoked potentials (ABR, stacked ABR, AMLR, ALR, ASSR, Electrocochleography)
  • Audiologic rehabilitation, including LACE program and rehabilitative classes
  • Comprehensive hearing testing
  • Earplug/swim mold dispensing of custom products, including earplugs for musicians
  • Facial nerve function monitoring
  • Hearing aid consultation/evaluation
  • Hearing aid dispensing
  • Hearing aid electroacoustic analysis and real ear probe-microphone testing

What to Bring

    Before the Visit: Referral consultation request form, authorization obtained by referring physician or provider (if applicable), clinical notes detailing reason for hearing test or specific problem to be addressed, insurance card (front and back).
    At the Visit: Two patient identifications (one with picture), insurance care, medication list, if documentation listed as required before visit (see column on left) was not provided, please have patient bring items at the time of visit.

Making an Appointment

To make an appointment, please call (415) 353-2101.

If you are a member of an HMO, please obtain written authorization from your HMO for the procedures you are expected to undergo. If the insurance company does not cover any or all of these procedures, or if pre-authorization for each procedure has not been obtained, you'll be expected to pay in full at the time of your appointment.

Parking

Parking at Mount Zion

Public parking is available in a parking garage at 1635 Divisadero St., which is open from 5:30 a.m. to 9 p.m., Monday to Friday. The garage entrance is on Sutter Street.

Metered street parking is rarely available.

Mount Zion

UCSF Medical Center at Mount Zion is accessible via the following Muni bus routes:

The No. 1 California bus route stops at the corner of California and Divisadero streets, which is three blocks north of Mount Zion.

UCSF Shuttles

Free UCSF shuttles travel between the Parnassus and Mount Zion campuses as well as other UCSF locations. For your convenience we have a Parnassus-Mount Zion Express Shuttle that leaves every 15 minutes.

The shuttle departs from Parnassus in front of the UCSF Kalmanovitz Library at 530 Parnassus Ave.

  • At Mount Zion, the shuttle departs in front of the UCSF Shuttle sign at the corner of Sutter and Divisadero streets.
  • The first shuttle of the day leaves Parnassus at 6:45 a.m.
  • The last shuttle of the day leaves Parnassus at 8:45 p.m.
