The Transfer Center is open 24-hours daily to coordinate the transfer of patients to UCSF Medical Center. The center can help you access our doctors, nurses, financial counselors, case managers, social workers and other members of our team. We evaluate the needs of each patient to ensure that appropriate care is provided.
Every year, we help facilitate the transfer of more than 2,500 patients, including women with high-risk pregnancies. About 10 percent of our hospital patients are admitted through the Transfer Center.
When your patient is discharged, the Transfer Center will facilitate the transfer of your patient back to your hospital or practice. We also will provide referrals and other information for continuum of care, if your patient requires services not offered at your facility.
