To refer a patient to EDAPP, call us at the number above during regular business hours. Then follow the referral submission instructions below. Once we receive your referral, a member of our support staff will contact the patient to schedule an appointment.

There are two options for submitting a referral:

1. Use MD Link, our secure portal for physicians.
2. Download our referral form, complete it and fax it to the clinic.
