To refer a patient to EDAPP, call us at (415) 353-2928 during regular business hours. Then follow the steps below to submit a referral form. Once we receive it, a member of our support staff will contact the patient to schedule an appointment.

- Authorization from insurance company, if needed
- Any relevant medical records or test results
