Medical Records

If you have a MyChart account, you can access most of your medical records there without having to make a special request.
For records that aren't available in MyChart – or if you don't use MyChart – you can request your records using any of the following methods. (For medical imaging or psychiatric care records, see the relevant sections below.)
- Request via MyChart. Log in to MyChart then go to Menu → Sharing Hub → Yourself → Request a copy.
- Send us a release form. Complete the health information release form and mail it to the address below. (Form for Spanish-speaking patients: Autorización de divulgación de información médica.)
- Send us a written request. Please include your medical record or unit number, your full name at the time of treatment, and your signature to authorize release of this information. Mail your request to the address below.
We'll process your request within 15 days and send you the records using the method you indicated. You also may arrange to pick up your medical records in person at any of our Medical Records locations.

Psychiatric records
All outpatient psychiatric care moved to the UCSF Nancy Friend Pritzker Psychiatry Building on June 27, 2022. To request your psychiatric care records, please follow the guidelines below:
- For outpatient care received before June 27, 2022, and for all inpatient care, contact UCSF Langley Porter Psychiatric Hospital to request your records.
- For outpatient care received at the UCSF Nancy Friend Pritzker Psychiatry building any time after June 27, 2022, contact the Pritzker building to request your records.

Processing fee
We charge 25¢ per page to copy medical records or $25 if using an electronic record. We'll call to notify you of the total charge. You may mail us a check or pay the fee in person if you pick up your records.

Health information exchange
A health information exchange (HIE) allows your health care providers to use secure, electronic methods to share your most recent medical information with doctors' offices, hospitals, labs, radiology centers and other providers participating in your care. The purpose is to give each of your caregivers access to the most up-to-date information available.
Patients are automatically set to opt in to HIE, meaning information will be shared with participating physicians and health care providers unless you provide instructions to opt out. If you wish to opt out of participating in UCSF HIE, you can do so when checking in for your next visit to UCSF or by submitting an opt-out request online:
