Refer a Patient: Nephrology (Kidney Care) Parnassus

Please note that your patient can schedule an appointment with us before the referral process is complete.

When submitting a referral, include the following for all patients:

  • Notes from the previous nephrologist, if the patient has seen one
  • Renal imaging and results (MRI, CT or ultrasound)

Please send additional records as needed for your patient:

  • Kidney stone prevention referrals: Lab tests, such as 24-hour urine studies and relevant blood work
  • General nephrology and PKD referrals: A comprehensive metabolic panel (BUN, creatinine, eGFR, CO2, serum potassium, serum sodium, and glucose) from within the past three months
  • General nephrology referrals: Dipstick urinalysis values from within the past three months

Please check whether the patient's insurance plan requires an authorization, RAF or PCP referral. If authorization is needed, use CPT code 99205. If no authorization/RAF/PCP referral form is required, please indicate this.
