Refer a Patient: Liver Disease Parnassus
Please note that your patient can schedule an appointment with us before the referral process is complete.
When submitting a referral, include the following:
- All related doctor's notes
- Results from all relevant lab tests
- Abdominal imaging tests (such as CT scans, MRIs or ultrasounds)
- Results of any gastroenterology procedures, such as colonoscopies or liver biopsies
- A copy of authorization from insurance, if required
If you’re referring a liver tumor patient, please send us:
- Pre-authorization for three new patient visits if the patient needs to see hepatology, surgery and interventional radiology specialists
- Alpha-fetoprotein (AFP) test
- Liver enzymes test
- Chemistry panel
- CBC blood test
- Prothrombin time (PT) test
- Lab work related to the cause of the patient's liver disease
- Most recent imaging tests (such as MRI and CT scans)
