You can request an appointment by filling out the form below. After we receive your form, we’ll contact you to schedule your visit.

Please use this form only if you:

• Are making an appointment with a provider you’ve never seen or haven’t seen in the past three years. Otherwise, schedule through our MyChart patient portal.
• Live in the United States. Patients outside the country should contact International Services  to arrange a visit to UCSF Health.
• Do not need immediate care. This form is for non-urgent health needs only.

Be sure to have a doctor’s referral and prior authorization if required by your insurance plan.
