Refer a Patient: Nutrition Counseling Parnassus
When sending a referral, please be sure to include:
- The ICD-9 codes for diagnoses that you wish the dietitian to address
- Pertinent medical records, including recent lab results, for patients not under the care of UCSF Health
Insurance coverage
Many insurance plans do not cover medical nutrition therapy. Plead inform your patient that they may need to pay out of pocket if their insurance company does not authorize nutrition counseling.
If your patient has Hill Physicians or Brown & Toland coverage, submit a request for authorization. For other insurance plans, instruct your patient to contact their insurance company to determine whether nutrition counseling is a covered benefit. Authorization should be confirmed before scheduling an appointment.
