Refer a Patient: Oral Oncology & Dysplasia Mission Bay
Please note that your patient can schedule an appointment with us before the referral process is complete.
When submitting an oral oncology referral, include the following:
- Recent consult notes
- Cancer treatment summary
- Recent pathology report
- Insurance authorization if applicable
If you’re referring an oral dysplasia patient, the following records are helpful but not required:
- Biopsy reports
- Clinical photographs
- Clinical notes, especially if treatments were performed
- Concerns or questions you’d like our team to address
