Request an Appointment: Speech-Language Pathology Mount Zion

Please complete the following steps to make an appointment:

1. Referral. Obtain a referral for speech-language pathology from a UCSF Health provider or affiliate. The date on your referral must be within the past six months.
2. Insurance authorization. Confirm that your insurance plan covers speech-language pathology at UCSF Health. If prior authorization is required, we will request approval for these services from your insurance company. (If your plan doesn't cover speech-language pathology at UCSF, you may be able to access this care at an in-network practice in your community.)
