Anthem Blue Cross PPO Enhanced Care Coordination

UCSF is working with Anthem Blue Cross to give you access to more information — and a whole team of health care professionals — to better support you in managing your health care needs and help you stay healthy.

Anthem Blue Cross PPO members who regularly receive care from UCSF for ongoing health services are welcome to participate in Anthem Blue Cross PPO Enhanced Care Coordination. Your benefit plan will stay the same, and there will be no change to your health care eligibility with Anthem Blue Cross. The only difference is that you will have access to an expanded team of health care professionals who will work directly with you and your primary care physician to develop a personalized health care plan and support you in meeting your health goals.

Your team at UCSF and Anthem Blue Cross will set up extra care when you need it and give you the support and resources you need most. Depending on your needs, your team may include:

Your UCSF doctor.
A Care Support Team member who will work with you to make managing your health care easier. (This is in addition to your doctor or other providers that you currently see.)
A care coordinator when you are at a rehabilitation facility or discharged to your home.
A clinical pharmacist who can assist you with understanding your medications.

Cigna PPO Collaborative Accountable Care

Collaborative Care focuses on your needs. As part of Collaborative Care, a UCSF nurse care coordinator works with you, your PCP and Cigna to help manage and coordinate the various aspects of your health care. This valuable resource is part of the services provided by UCSF at no added cost to you. You are automatically enrolled with no action required on your part.

Blue Shield HMO ACO – City and County of San Francisco

You are a member of this ACO if you are an employee of CCSF (or a dependent of a CCSF employee) and have BSC HMO as your health insurance and a Hill Physician primary care physician in San Francisco.

All UCSF primary care physicians are part of Hill Physicians Medical Group.

Additional partners in this ACO include the Dignity Hospitals and providers in San Francisco (St. Mary’s/St. Francis).

Blue Shield HMO ACO

You are a member of this ACO if you have BSC HMO as your health insurance, you are insured through your employer (that is, you are not a Medicare patient) and have selected a Hill Physician primary care physician in San Francisco.

All UCSF primary care physicians are part of Hill Physicians Medical Group.

Health Net Blue & Gold ACO - Canopy

You are a member of this ACO if you are a UC employee, dependent, spouse or UC retiree not eligible for Medicare and have Health Net Blue & Gold Canopy as your health insurance and have selected a San Francisco Hill Physicians Medical Group provider as your primary care provider.
