Therese Chan Tack, DO, MPH


Dr. Therese Chan Tack is an internal medicine physician at UCSF Primary Care at China Basin, where she provides primary care for adults. She collaborates with her patients in providing hands-on, holistic preventive care and chronic disease management.

Chan Tack received her medical training at the Chicago College of Osteopathic Medicine. After finishing both residency and chief residency in internal medicine at Loma Linda University Medical Center, she completed a Master of Public Health degree at UC Berkeley. She practiced both primary care and hospital medicine with Santa Clara Valley Medical Center for three years. She is an assistant clinical professor of internal medicine at UCSF.
