1. Send required documents

Please fax us the following:

• Referral form: Be sure to include ICD-9 codes for diagnoses that you wish the dietitian to address.

  Download referral form

• Pertinent medical records, including recent lab results, for patients not under the care of UCSF Medical Center

2. Do an insurance check

Nutrition counseling for medical nutrition therapy is often not covered as an insurance benefit. Patients should be informed that they may be required to pay if their insurance does not authorize nutrition counseling.

If your patient has Hill Physicians or Brown & Toland coverage, submit a request for authorization. For other insurance plans, instruct patients to contact their insurance company to determine whether nutrition counseling is a covered benefit. Authorization should be confirmed before scheduling an appointment.

3. Schedule an appointment

Once you've sent us the referral and verified insurance coverage, ask your patient to schedule a nutrition counseling appointment.
